It's true that "The Normal Heart," Larry Kramer's fiery 1985 docu-like drama about the AIDS pandemic, suffers from agit-prop agita, even in this first-rate production starring Joe Mantello and Ellen Barkin and co-helmed with enormous heart by Joel Grey and George C. Wolfe. But Kramer's is a fine fury, stoked by righteous anger, and even today the sobering specifics of his impassioned brief are worth recall. Especially if, as the scribe declares in printed fliers that he sometimes distributes himself after the show, AIDS is still unacknowledged as "a worldwide plague" with "no cure" and little coordination in its treatment."
The heart has an automatic conduction system that signals each beat. This inherent pacemaker system is called the sinus node located within the right atrial wall. The sinoatrial node produces an electrical impulse that travels to a central conduction site called the AV node and is then sent down to the ventricular muscle. This in turn causes a contraction of the ventricular muscle ejecting blood to both the lungs and the aorta. The number of contractions each minute is what is referred to as the heart rate or pulse. In newborns and young children, this heart rate is much faster (100-170 beats/minute) than in older children or adults (70-100 beats/minute).
Some people will have a heart rate of 90 or even 100 bpm while they are at rest. This may be normal for that person if he or she feels well. More commonly, faster heart rates are due to anxiety, an overactive thyroid, lung disease or medications. Heart rates faster than 100 bpm at rest are usually abnormal. A rate that fast usually needs evaluation by a heart rhythm specialist if the usual causes (anxiety, overactive thyroid, etc.) have been ruled out. Inappropriate sinus tachycardia, supraventricular tachycardia and postural orthostatic tachycardia are some of the common causes of fast heart rates that require diagnosis and treatment.
